Initial Establishment License Fee $100.00

Establishment Renewal Fee $50.00

Initial Massage Therapy School Registration Fee $50.00

Massage Therapy School Renewal Fee $10.00
Massage Therapy Instructor License Fee $10.00

License Reactivation Fee $75.00 if license is expired for more than 1 year and all applicable renewal fees

Late License Fee $25.00 if received after the expiration date.

License Verification Fee $15.00
Duplicate License Fee $10.00

Copy of roster of Alabama license massage therapists: thirty dollars ($30),

Copy of roster of approved massage therapy schools: five dollars ($5),

Copy fee per page: twenty five cents ($.25),

Postage and handling: actual postage plus one dollar fifty cents ($1.50) per item handling.

All fees are nonrefundable.

All fees must be submitted in the form of a cashier’s or certified check, or money order.
No personal, Business checks or cash will be accepted.
